Passport: a valid passport which has at least 6 months validity and tow blanket pages.
Visa application form: this form can be downloaded from Thailand embassy website in Pakistan, it should be printed and filled and signed by the applicant.
Photos: 2 passport sized photos with white background, not glass, taken within last 6 months.
Proof of accomodation: an address in Thailand required, where you will stay in your trip to Thailand, this can be hotel reservation, rental agreement or something similar.
Proof of Financial stability: you need to provide bank records, payslip or any other official documents which shows you have enough funds to support yourself during your visit to Thailand.

Confirmed round trip ticket: one the most important requirements is a round trip paid ticket, you should buy a ticket for the date you travel to Thailand, your ticket must be for both sides, your trip to Thailand and return back to Pakistan.
Visa fee payment: you should pay the visa fee which can be 12000 pkr.

You can apply directly throug embassy or through authorized travel agencies.
